Insurance & Payment Policy


As our practice and physician do not file claims or have contracts with any commercial or government insurance plans, we do require all payments to be provided at the time of service, in the form of a major credit card, debit/ATM card or cash. We apologize but checks and CareCredit are not accepted at our practice.

If you are covered under an insurance plan with out-of-network benefits, we will provide you with an itemized billing statement consisting of all the necessary billing codes for you to file on your own for possible reimbursement. Please contact your customer service department on your health insurance card for further verification of the type of coverage you may have and your plan’s reimbursement policy.


Many of our patients use their healthcare reimbursement/flexible spending credit card for payment of medical services. Please verify with your plan the amount available on your card prior to your visit. We accept these cards as long as they carry a Visa, MasterCard, Discover or American Express logo. If sufficient funds are not available on your card, we will require another form of payment, such as a major credit card, debit/ATM card, or cash. Please note that checks are not accepted.
