Please arrive 15 minutes before your initial appointment to provide essential patient information. To enhance your care, registration can also be done prior to the visit by selecting the "New Patient Registration Forms" option from this website.  For security purposes, we ask you to bring a valid/active photo identification card at your first visit (i.e. driver's license, state ID card, military ID, passport).

All patients under the age of 18 must be accompanied by a parent, a guardian or an adult designated by the parent with written permission.  For safety reasons, we ask parents to not bring children, unless they are being seen as patients. We at Esthetics Center for Dermatology want to provide you and others with the dedicated, uncompromised care you all deserve.  

Please refrain from the use of cellular phones/pagers/PDAs in the building.  This is due to safety, as well as courtesy.   Please be sure to turn off your cell phone/pager/PDA ringer or put it on a silent/vibrate setting.  If the call is urgent, please conduct the conversation outside the building.  We sincerely thank you for your cooperation.
